Cancellation Policy
This agreement is made between: Jason Brown, Certified Personal Trainer and client: The Trainer will provide personal training sessions, which may include virtual sessions (conducted via video conferencing platforms such as Zoom) and/or in-person sessions (at [specify location, e.g., gym, client's home, or other agreed venue]). Services may include, but are not limited to: Customized workout plans. Nutritional guidance (non-medical advice only). Fitness assessments and progress tracking. Motivational coaching. Sessions will be scheduled by mutual agreement. Each session length as stated in package purchase. Client Responsibilities The Client agrees to: Provide accurate health and medical history information prior to starting sessions, including any pre-existing conditions, injuries, or medications. Consult with a qualified healthcare professional before beginning the program to ensure they are physically able to participate. Follow the Trainer's instructions during sessions and promptly notify the Trainer of any pain, discomfort, or changes in health. For in-person sessions: Arrive on time, wear appropriate attire, and maintain a safe environment (e.g., no hazards at the session location). For virtual sessions: Ensure a safe workout space, use equipment properly, and not record sessions without written consent. Comply with all applicable laws and not hold the Trainer responsible for the Client's failure to do so. The Client acknowledges that personal training involves inherent risks, including but not limited to muscle strain, injury, or exacerbation of pre-existing conditions, and that results are not guaranteed. 4. Waiver and Release of Liability Assumption of Risk: The Client understands and voluntarily assumes all risks associated with participating in the Trainer's services, including but not limited to physical injury, illness, death, property damage, or loss arising from: Exercise activities (e.g., weightlifting, cardio, stretching). Use of equipment (provided by Trainer or Client). Virtual sessions (e.g., improper form due to remote instruction). Environmental factors (e.g., uneven surfaces, weather for outdoor sessions). Any other foreseeable or unforeseeable risks. The Client represents that they are in good health and have no medical conditions that would prevent safe participation. If any health issues arise, the Client agrees to immediately cease participation and seek medical advice. Release of Liability: In consideration
